UHC 25: Some Physician Stories in Hospital ERs, Wards

Below are notes by a physician friend, Doc Erwin Abueva, in his facebook wall, February-March this year. Real stories from physicians in emergency room (ER), wards, operating rooms. Some stories are funny, some are sad. Somehow they provide additional dimensions in the continuing discussions about universal health care (UHC). Thanks a lot for sharing these stories Doc. Non-physicians like me find them informative.

(1) February 6
2PM, HEMODIALYSIS Unit, Private Tertiary Hospital

Two male dialysis patients, both in their late 50's, were talking to each other. One patient is having his dialysis for 3 years already in the said hospital, while the other is a "first-timer", had his dialysis sessions in a hospital in Bulacan for 2 years.

PATIENT 1: Bago ka po dito, no? Ngayon lang kita nakita dito. (smiling).
PATIENT 2: Opo. Sa Bulacan po ako dati nagdadialysis. Lumipat na ako dito kasi sa anak kong panganay na ako nakatira. Sa kanila na lang daw kami ng asawa ko.
P1: Bakasyon?
P2: Hindi po. Naibenta ko na kasi yun bahay namin sa Bulacan dahil sa pagdadialysis. Ang hirap ng sakit natin, ubos ang pera. Ang mga naipundar kong konti unti-unting nawala na.
P1: Talagang mahirap. Mabuti na lang medyo marami pa ako savings.
P2: Buti pa po kayo. Maraming panggastos. Swerte nyo po.
P1: Ikaw ang swerte. Kasi ikaw kahit wala ka nang pera, nariyan ang anak mo nakasuporta. Ang asawa mo kasama mo ngayon, hindi ka pinapabayaan. Ako, iniwan na ako ng asawa ko last year. Pumunta na sa Amerika. Napagod na yata sa pag-aasikaso sa akin. Ang mga anak ko parehong nasa abroad. Ni tawagan ako hindi nila magawa. driver ko lang kasama ko lagi dito sa dialysis.
P2: Opo, kasama ko pamilya ko. Pero wala naman akong pangsustento sa dialysis ko. Hindi din ganon kaswerte.
P1: Tandaan mo, lahat naman tayo mamamatay din. Ang tanong lang e sinong nasa tabi mo sa huling sandali ng buhay mo...

(2) February 11
6AM, EMERGENCY ROOM, Private Tertiary Hospital

A 64 y.o. Male came in due to nose bleeding. With him was his wife and his daughter.
While the nurses are packing the patient's nostrils with gauze...

DOCTOR: Ano pong nangyari sa inyo sir? Bakit dumugo ang ilong nyo?
PATIENT: Napasubasob ako sa sahig. Mukha ko tumama diretso sa sahig.
DOCTOR: Paano pong napasubasob? Nadapa po kayo o natisod ba? Buti di kayo nawalan ng malay o napasama ang dapa.
PATIENT: Hindi naman ako natisod o nadapa doc. Napasubasob lang basta.
WIFE: Naku doc, ganito kasi yan. Pagkabangon nya sa kama, lumuhod daw sya para magdasal. Tapos bigla na lang natumba sa pagkakaluhod at tumama yun mukha sa sahig.
DOCTOR: Ibig sabihin nakatulog kayo habang nagdadasal kaya napasubasob kayo?
PATIENT: parang ganun nga yata nangyari sa akin.
DAUGHTER: Hay naku. Tatay, next time kasi huwag na kayo lumuhod pag magdadasal kayo. Pwede naman nakahiga kung magdadasal.
WIFE: At huwag mong masyadong habaan ang dasal mo para di ka antukin.

(3) February 17
3:30AM, Emergency Room of a Private Hospital
A 24 y.o. Female patient came and directly went to the Doctor's Table.

PATIENT: Sino doctor? I need to see a doctor.
DOCTOR: Yes ma'am. I'm the doctor here.
PATIENT: Bothered kasi ako.
DOCTOR: Sige po ma'am. Take a seat. Ano ba ikoconsult mo at napasugod ka sa ER ng 3AM na?
PATIENT: kasi eto o. An-an ba ito doc nasa leeg ko? Di kasi ako sure.

(4) February 22
An 11 y.o. female patient was brought by relatives due to asthma attack. He was exposed from dust while her mother was cleaning their cabinet.

DOCTOR: (to the mother) paiinjectan po natin ng antihistamine sa arm nya. And magbibigay din ng steroid thru IV. Sa ugat po papadaainin yun gamot. Magnebulize din po ang bata ngayon.
MOMMY: Sige doc kung yun ang dapat. Ok lang po.
FATHER: Ikaw magtutusok (looking at the nurse)? Galingan mo sa pagtusok. Pag pumalya yan, makikita mo.
NURSE: Sir, itatry po namin na 1 shot lang.
Unfortunately, the nurse missed his first try.
LOLO: Tsk tsk. Wala bang sharp shooter dito? Hindi ka naman pala marunong e. Yun sigurado. Yun magaling.
DOCTOR: Pasensya na po pero minsan mahirap maginsert. I understand ayaw nyo masaktan ang anak nyo. Ang mga nurses po dito experienced na maglagay ng swero. Pero wala pong hindi nagkakamali.
Another nurse tried inserting heplock but was also failed.
FATHER: Tsk tsk. Sinasaktan nyo lang ang anak ko e. Siguraduhin mo nurse! Huwag kang magtry kung di mo siguradong mashushoot mo. Ikaw Doc, ikaw na lang para 100% sure sa pagtusok
DOCTOR: Sir, kung 100% sure ang hanap nyo, hindi ko din magaguarantee yan. Sa totoo lang, mas experienced pa ang ER nurses namin sa paglagay ng swero kesa sa akin. Pwede po bang kami na lang muna at si mommy nalang dito?
MOTHER: Oo nga. Umalis na muna kayo Tatay (lolo). Doon muna kayo sa labas.
After the two men went out, the same nurse was able to insert "heplock" to the patient's hand, fortunately with 1 try only. Swabeng tusok, walang pressure.
LOLO: Buti nalagyan na.
MOTHER: Kanina pa dapat kung hindi kayo mga masusungit sa nurse.

(5) February 25
Several minutes ago, a 65 y.o. male was rushed to the ER due to loss of consciousness. The patient was with his wife strolling at SM Marikina when he suddenly fell on the floor unconscious.

The patient had 3 previous heart attacks and underwent open heart surgery few years ago.
The patient did not respond to CPR done, to all medications given. He was pronounced dead 40 minutes after.
Sad events like this makes me realize of my mortality.
Few minutes ago nag-uusap lang sila and nagtatawanan. In a matter of seconds, nawala na ang isa, permanently!
Maybe mas maraming nauunang lalaki kesa sa babae. There are more widows than widowers. I just hope when my time comes, hindi sudden, hindi biglaan... 

(6) February 28
At the WARDS, Tertiary Private Hospital

A 67 y.o. Male patient was pronounced dead by the doctor after 30 minutes of CPR. The patient suffered from a Heart Attack.
His wife, his son and daughter-in-law, and many of his grandchildren were there.

WIFE: Wala na po ba talaga ang asawa ko?
DOCTOR: Misis, I'm sorry. Hindi na po nagrespond ang katawan nya sa mga gamot na ibinigay namin. Nakikiramay po kami.
SON: Ma, pa'no natin ipapaalam kina Ate at Kuya na wala na Papa? Sino tatawag sa Australia? Dapat kasi noong naconfine si Dad dito last week sa e ininform na natin sila.
WIFE: Ayoko na kasi sila mag-alala sa papa nyo pag malaman nilang naospital. Hindi ko naman akalain na ganito mangyayari sa papa nyo. Ang bilis naman!
SON: (approached the corpse of the patient) Papa, sige Magrest ka na. Kami na bahala kay mama. Huwag mo kami alalahanin... Siguradong uuwi na mga kapatid ko. Makukumpleto na uli tayo papa after 5 years.... (crying)

(7) March 5
1PM, OPERATING ROOM, Tertiary Gevernment Hospital

An OB-Gynecologist was doing a Caesarian Delivery due to "failure of induction of labor". Patient had 14 hours of labor prior to surgery. After the baby came out and the placenta removed, the uterus kept on bleeding as it remained soft (nor contracted).

OB: Doc (talking to the anesthesiologist), pakidagdagan ang oxytocin to 20 units. Malambot pa rin ang uterus ng patient.
ANESTH: Sige Doc.
OB: (talking to the circulating nurse) Ma'am, may cytotec ba tayo?
NURSE: Meron po. Tingnan ko lang po sa cabinet Doc.
After 2 minutes, the nurse returned.
NURSE: Doc, sorry. Wala na po pala. Ginamit po kahapon ni Dra._______ sa patient nya.
OB: wala na talaga? Pano yan? Malambot pa din. Ayaw tumigas ng matris! Ok, sige magpray na lang tayo lahat. Makukuha din yan sa prayers. Magdasal na lang tayong lahat, ok?
After 20 minutes, while starting to close the layers of abdominal wall.
OB: Doc mukhang tumigas din ang matris. Tumigil na ang bleeding oh! Kita nyo, 1 "Our Father" at 1 "I Believe" lang ang kelangan.
PATIENT: (mildly sedated) Doc, nagdadasal ka?
ANESTH: (surprised at the talking patient) Gising ka pa pala? Sorry Doc (looking at the OB).
OR ASSIST: Boom!

(8) March 10
9:30AM, EMERGENCY ROOM
A 45 y.o. female came in due to "nervous breakdown". 

DOCTOR: misis, relax lang po. Huwag pong maghabol ng paghinga lalo kayo mahihirapan nyan.
RELATIVE: Doc, nakipag-away po yan sa kapitbahay. Sa sama ng loob at tindi ng galit, nagcollapse! Tapos iyak ng iyak. Ngayon ang bilis huminga.
DOCTOR: sige po. Pabibigyan ko po ng pampakalma. May sakit po ba sya? Hika? Diabetes? Allergy sa gamot o pagkain?
RELATIVE: wala po.
DOCTOR: May "Highblood" po ba sya?
RELATIVE: Anemic po kami. Low blood, doc.
DOCTOR: ha?

(9) March 18
CAFETERIA, Private Tertiary Hospital
Around 10 people were in line for their turn to order food. A doctor came in and went to the back of the last person.

CUSTOMER 1: Doc, dito na po kayo. Mahaba pila. Huwag na po kayo pumila.
CUSTOMER 2: Ano ba yan?! Porke doctor pwede na lang sumingit? (in loud voice)
CUSTOMER 1: Huwag kang mag-alala (talking to the complaining customer) Ibibigay ko lang yun pwesto ko sa pila para kay Doc. Ako ang pupunta sa hulihan.
DOCTOR: Huwag na ok lang ako. Huwag ka na umalis jan.
CUSTOMER 1: Doc, di pa kayo naglalunch alam ko. Baka nahihilo na kayo.
DOCTOR: Oo nga e. Dami kasi pasyente sa clinic. Pero ok lang. Kaya pa.
CUSTOMER 2: Mabuti naman at di na sumingit! (in loud voice)
After an hour, at the Clinic.
DOCTOR: Next patient please. Mr. _______, kayo na po.
CUSTOMER 2: (approached the Doctor) Doc, pwede bang yun nanay ko muna? Nahihilo na daw kasi.
DOCTOR: Kanina parang ang hirap para sa'yo na magbigay sa pila sa canteen. Natatandaan mo ako?
CUSTOMER 2: Ahhhh. Sige Doc. Mamaya na lang kami.
DOCTOR: No. Sige ilapit mo ang nanay mo sa akin at sya na muna uunahin ko. Sir, ang tawag dyan ay konsiderasyon.

(10) March 24
10AM, Wards, Tertiary Government Training Hospital

During patient rounds at the Private Ward, resident physician with the Consultant.
RESIDENT: Mam, the patient was asking if he can eat halo-halo and sisig. Sabi ko huwag muna (NPO) until magrounds po kayo sa kanya.

CONSULTANT: Doc, bakit di mo pinakain? You should have started diet nung maconfind sya kagabi.
RESIDENT: Mam, Ano diet po nya? Start natin Low Salt, Low Fat, Low sugar Diet?
CONSULTANT: Kahit ano pwede nya kainin. Diet as tolerated.
RESIDENT: Mam, hypertensive and diabetic po patient.
CONSULTANT: Alam ko... Doc, I know your point and hindi naman mali ang diet na pinopropose mo. But then, try to look at the bigger picture. How old is my patient?
RES: 89 y.o. na po.
CON: Bakit daw nagpaconfine?
RES: Nanghihina daw po sya. Hindi makatulog. Hindi makakain.
CON: Ano sakit nya?
RES: Diagnosed po with Prostate CA.
CON: Pakainin na natin ng kung ano gusto nya. Kung ano yun mag-eenjoy yun matanda. Kawawa naman kung low salt-low fat-low sugardiet pa ibibigay mo. Walang lasa yun! He's 89 y.o. already, suffered already for a long time due to his illness. Huwag na natin dagdagan ang suffering nya by giving foods na kahit ikaw hindi mo magugustuhan. For me, what's more important now is to alleviate his suffering. Matanda na sya. Ibigay na natin ang gusto.
RES: Ok po mam.
CON: do you know the film " Patch Adams"? I suggest you watch it. Mas maiintindihan mo ako.....

(11) March 31
EMERGENCY ROOM, Tertiary Private Hospital

A 65 y.o male was intubated at the ER due to cardiorespiratory distress. The patient suffered from heart attack.

DOCTOR: Misis, maaring mamaya lang e tumigil bigla ang puso nya. Masama talaga ang kondisyon ng puso. Pwede natin sya ilipat sa Heart Center. May mga baradong ugat sa puso na kelangan maopera.
MISIS: Doc, sa ngayon halos wala na kaming pera. Labas-pasok na din yan asawa ko sa ospital dahil sa puso nya. Noon inadvised na din sya na magpa-open Heart surgery pero sya mismo ang may ayaw.
DOCTOR: Siguro po dati medyo ok pa nararamdaman nya. Pero ngayon kelangan na talaga.
MISIS: Doc, kung ipapaoera namin sya sa puso, mauubos ang natitira na lang naming pera. Kawawa naman ang anak namin kung maubos sa kanya. Siguro hindi nya rin gustong maubos ang pera lahat namin para sa kanya.
DOCTOR: Misis, parang ang hirap ng pinagdadaanan nyo pala.
MISIS: Mahirap talaga Doc. Asawa ko yan. Mahal na mahal ko yan. Pero ganyan talaga ang buhay, di ba?
